Metropolitan Health Plan (MHP) was a health maintenance organization (HMO) based in Minneapolis, Minnesota, that specialized in providing managed care services for public program enrollees. Founded in 1983 and operated by Hennepin County, the plan served residents enrolled in Medicaid, MinnesotaCare, and other state-subsidized health programs. In 2016, MHP merged with Hennepin Health to create a more integrated system of care for the county's most vulnerable populations. The organization focused on improving health outcomes through coordinated care and community-based health initiatives. It was notable for being the first county-owned and operated HMO in the United States.
