NavCare

NavCare was a U.S. healthcare services and digital health company focused on virtual chronic care management, transitional care management, remote patient monitoring, telehealth, and care coordination for provider organizations. The company operated a clinician-led care center and used its proprietary NavCare Connect platform to support proactive assessments, vital-sign tracking, video e-visits, and ongoing patient engagement. NavCare appears to have originated within US CareNet and later operated as a distinct care management brand serving physician practices and other healthcare providers nationwide. In August 2023, MD Revolution acquired NavCare to expand its remote care management capabilities, and NavCare teams were integrated into MD Revolution. As a result, NavCare is best characterized today as an acquired, no-longer-independent healthcare services business rather than a therapeutics developer.
