RMS Disease Management, Inc. was a Chicago-based healthcare company specializing in chronic disease management programs for health plans and self-insured employers. The company focused on high-risk, high-cost populations, particularly those suffering from congestive heart failure, diabetes, and coronary artery disease. By utilizing proprietary clinical algorithms and telephonic nursing support, RMS aimed to reduce hospitalizations and improve patient quality of life. In 2006, the company was acquired by Matria Healthcare for approximately $115 million, eventually becoming part of Alere and later Abbott Laboratories.
