Scottsdale Health Partners (SHP) is a clinically integrated network and Accountable Care Organization (ACO) based in Scottsdale, Arizona. Founded in 2012 as a joint venture between HonorHealth and independent community physicians, the organization focuses on population health management and value-based care delivery. SHP provides the infrastructure, data analytics, and care coordination services necessary for healthcare providers to improve clinical outcomes while managing the total cost of care. The network includes hundreds of primary care and specialty physicians serving the Phoenix metropolitan area. It participates in various risk-based contracts, including the Medicare Shared Savings Program.
