South Devon Healthcare NHS Trust was a National Health Service organization responsible for managing Torbay Hospital and providing acute healthcare services to the population of South Devon, England. Established as an NHS Trust in 1991, it became one of the first NHS Foundation Trusts in 2007, gaining greater financial and operational autonomy. The trust was a pioneer in integrated care, developing the internationally recognized Torbay Model to streamline patient transitions between hospital and community settings. In October 2015, it merged with Torbay and Southern Devon Health and Care NHS Trust to form the Torbay and South Devon NHS Foundation Trust. This merger created one of the UK's first Integrated Care Organisations, aimed at providing seamless health and social care services.
