Triad Group and its sister company, Triad Pharmaceuticals, were prominent manufacturers of over-the-counter (OTC) medical products, specializing in antiseptic supplies such as alcohol prep pads, swabs, and povidone-iodine products. Based in Hartland, Wisconsin, the company operated as a contract manufacturer and distributor for major retail brands. In 2011, the company became the center of a massive federal investigation and nationwide recall after its products were found to be contaminated with Bacillus cereus, a bacterium linked to several serious infections and deaths. Following the recall and subsequent legal actions by the FDA and Department of Justice, the company ceased all manufacturing operations and filed for bankruptcy. The case remains a significant case study in pharmaceutical quality control and regulatory compliance for the healthcare industry.
