Vermont Care Partners

Vermont Care Partners is a non-profit statewide network of 16 community-based agencies providing mental health, substance use, and intellectual and developmental disability services across Vermont. The organization serves as a unified voice for its member agencies, focusing on advocacy, policy development, and the implementation of high-quality, integrated care models. Through its Vermont Care Network and Vermont Council branches, it facilitates data-driven quality improvement and statewide health information technology initiatives.
