αFAP-PE38 is a recombinant immunotoxin composed of variable regions of a monoclonal antibody targeting fibroblast activation protein (FAP), fused to a truncated form of Pseudomonas exotoxin A (PE38). It binds specifically and with high affinity to FAP, a protein preferentially expressed on tumor-associated stromal fibroblasts in a variety of epithelial cancers. Upon binding FAP, αFAP-PE38 is internalized and delivers the PE38 toxin to the cytosol, resulting in selective cytotoxicity of FAP-expressing cells. In preclinical studies, αFAP-PE38 inhibited tumor growth in mouse models and altered the tumor microenvironment by reducing angiogenic, chemotactic, and immunosuppressive factors.[1][3]
