εPA44 is a therapeutic vaccine candidate developed by the First Hospital of Peking University for the treatment of chronic hepatitis B (CHB). It is a recombinant fusion protein that combines the Hepatitis B virus e antigen (HBeAg) with the translocation domain (Domain II) of Pseudomonas aeruginosa exotoxin A (PEA). The vaccine is designed to overcome the immune tolerance characteristic of CHB by utilizing the PEA domain to facilitate the entry of HBeAg into the MHC class I antigen processing pathway of antigen-presenting cells. This mechanism aims to stimulate a robust HBeAg-specific CD8+ T-cell response, which is typically deficient in chronic carriers, to help clear the virus. Clinical trials have evaluated its efficacy in inducing HBeAg seroconversion and reducing HBV DNA levels in patients with chronic hepatitis B.
