Overview

Warfarin is an oral anticoagulant that exists as a racemic mixture of two enantiomers, (R)-warfarin and (S)-warfarin. The drug is primarily used to prevent and treat venous thrombosis, pulmonary embolism, and thromboembolic complications associated with atrial fibrillation and myocardial infarction. Warfarin acts by inhibiting the vitamin K epoxide reductase complex subunit 1 enzyme, thereby blocking the recycling of vitamin K. This inhibition reduces the synthesis of active vitamin K-dependent clotting factors II, VII, IX, X as well as regulatory proteins C and S. The R-enantiomer is less potent than S-warfarin but has a longer half-life; both contribute to the overall anticoagulant effect[1][2][8]. Warfarin was originally developed as a rodenticide before its medical use.
