(S,R)-fidarestat is a stereoisomer of fidarestat, a small molecule inhibitor of aldose reductase. Aldose reductase is the first enzyme in the polyol pathway and plays a key role in the pathogenesis of diabetic complications by catalyzing the reduction of glucose to sorbitol. Inhibition of this enzyme reduces sorbitol accumulation in tissues and has been investigated as a therapeutic strategy for diabetic neuropathy. Fidarestat selectively binds to aldose reductase with high affinity and shows minimal off-target effects or direct pharmacological actions beyond AR inhibition. Clinical studies have shown that fidarestat can improve nerve conduction velocity and subjective symptoms associated with diabetic neuropathy without significant adverse effects or laboratory abnormalities[1][2][5]. The compound belongs to the hydantoin class of organic compounds.
