111In-DOTA-daratumumab is a radiolabeled monoclonal antibody used as a diagnostic imaging agent in the management of multiple myeloma. It consists of the anti-CD38 monoclonal antibody daratumumab conjugated to the chelator DOTA (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid), which coordinates the gamma-emitting radioisotope indium-111. In clinical settings, it is primarily utilized for SPECT (Single Photon Emission Computed Tomography) imaging to evaluate the biodistribution, pharmacokinetics, and organ dosimetry of daratumumab-based therapies. It often serves as a companion diagnostic for alpha-emitting therapeutic radiopharmaceuticals, such as 225Ac-DOTA-daratumumab, in a theranostic approach to treat relapsed or refractory plasma cell malignancies.
