111In-DTPA-anti-PD-L1 is a radiopharmaceutical imaging agent consisting of a monoclonal antibody targeting programmed death-ligand 1 (PD-L1) conjugated to the chelator diethylenetriaminepentaacetic acid (DTPA) and labeled with the gamma-emitting radioisotope indium-111 (111In). Developed by researchers at Johns Hopkins University, this tracer is designed for single-photon emission computed tomography (SPECT) imaging to non-invasively assess PD-L1 expression levels in tumors. PD-L1 is a key immune checkpoint protein often overexpressed by cancer cells to evade immune detection; imaging its expression can help identify patients likely to respond to anti-PD-L1/PD-1 immunotherapies. Preclinical studies in melanoma models have demonstrated its ability to identify PD-L1-positive tumors and highlighted the role of the spleen as a significant sink for the tracer, which can impact its biodistribution and tumor uptake.
