111In-labelled DOTA-labetuzumab-IRDye800CW is a dual-modality imaging agent designed for the detection of carcinoembryonic antigen (CEA)-expressing tumors, particularly in colorectal cancer. It consists of labetuzumab, a humanized monoclonal antibody targeting CEA, conjugated with both the near-infrared fluorescent dye IRDye800CW and the radioactive isotope Indium-111 via a DOTA chelator. This combination enables both preoperative single-photon emission computed tomography (SPECT) imaging and intraoperative near-infrared fluorescence (NIRF) guidance for tumor localization and resection. The agent has demonstrated safety and feasibility in phase II clinical studies for patients with peritoneal metastases from colorectal cancer undergoing cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS-HIPEC). Its mechanism relies on specific binding to CEA on tumor cells, allowing sensitive detection of malignant lesions that may be difficult to visualize by conventional means[6][8][4][5].
