123I-CMICE-013 is a novel radiotracer developed for Single-Photon Emission Computed Tomography (SPECT) myocardial perfusion imaging (MPI). It is a rotenone derivative that functions as a mitochondrial complex I (NADH dehydrogenase) inhibitor. The tracer demonstrates promising characteristics for cardiac imaging with high myocardial uptake (1.5% of injected dose at rest) and favorable heart-to-background ratios. Studies show that 123I-CMICE-013 uptake increases more rapidly with increased blood flow compared to standard SPECT tracers like tetrofosmin, sestamibi, and 201Tl. The tracer has a molecular formula of C23H23IO7 and is administered intravenously.
