123I-cT84.66 is a radiolabeled chimeric monoclonal antibody developed for the diagnostic imaging of tumors expressing Carcinoembryonic Antigen (CEA). The molecule consists of the cT84.66 antibody, a chimeric version of the murine monoclonal antibody T84.66, which has been engineered to reduce immunogenicity while maintaining high affinity for CEA (CEACAM5). CEA is a cell-surface glycoprotein that is significantly overexpressed in various solid tumors, most notably colorectal, gastric, and pancreatic carcinomas. By labeling the antibody with Iodine-123, a gamma-emitting radioisotope, the agent enables the non-invasive localization of CEA-positive lesions using Single Photon Emission Computed Tomography (SPECT). This diagnostic tool is primarily utilized to assess the extent of metastatic disease, identify candidates for subsequent radioimmunotherapy, and monitor treatment response in patients with gastrointestinal malignancies.
