123I-DualFAPi-5 is a diagnostic radiopharmaceutical developed by TetraKit Technologies for the imaging of fibroblast activation protein (FAP)-expressing tumors. It consists of a FAP inhibitor (FAPi) dimer (DualFAPi-5) labeled with the gamma-emitting radioisotope iodine-123 (123I) using TetraKit's proprietary tetrazine-based click chemistry platform. Designed to prolong tumor residence time and improve tumor-to-background ratios, 123I-DualFAPi-5 is utilized as a diagnostic imaging agent for single-photon emission computed tomography (SPECT) to detect FAP-expressing tumors, such as triple-negative breast cancer (TNBC). It is being developed as a diagnostic companion or imaging counterpart to therapeutic radiohalogenated agents like 131I-DualFAPi-5 and 211At-DualFAPi-5.
