123I-Iodometomidate is a radiopharmaceutical imaging agent used for adrenal scintigraphy. It is a radiolabeled derivative of metomidate, a potent inhibitor of the steroidogenic enzymes CYP11B1 (11ß-hydroxylase) and CYP11B2 (aldosterone synthase). These enzymes are expressed exclusively in the adrenal cortex, making iodometomidate a highly specific tracer for adrenocortical tissue. It is primarily used to differentiate between adrenocortical and non-adrenocortical tumors, such as in the diagnosis of adrenocortical carcinoma or the localization of aldosterone-producing adenomas. The development of this tracer has been significantly led by researchers at the University of Wuerzburg, particularly for the evaluation of adrenal incidentalomas and the staging of adrenocortical cancer.
