123I-labeled anti-CEA Fab is a diagnostic radiopharmaceutical consisting of the Fab fragment of a monoclonal antibody specific for carcinoembryonic antigen (CEA), radiolabeled with the gamma-emitting isotope Iodine-123. Developed for the localization of primary, recurrent, and metastatic colorectal carcinoma, it is used in conjunction with emission-computerized tomography (ECT). The use of the Fab fragment, which lacks the Fc portion of the antibody, allows for significantly faster blood clearance and reduced non-specific background signaling compared to intact IgG or F(ab')2 fragments. This pharmacokinetic profile enables high-contrast tumor imaging as early as 6 to 24 hours post-injection, facilitating the detection of small tumor masses and recurrences even in patients with normal circulating CEA levels.
