124I-18B10 is an investigational **radiolabeled humanized monoclonal antibody** PET imaging agent designed for gastrointestinal tumors that overexpress Claudin 18.2. It consists of the humanized anti-CLDN18.2 antibody 18B10 labeled with iodine-124, enabling noninvasive positron emission tomography visualization of CLDN18.2-positive lesions. The agent has been studied in a phase 0 setting as a molecular imaging biomarker to identify patients who may benefit from CLDN18.2-targeted therapies and to support efficacy evaluation; thyroid blockade with iodine is used during imaging to reduce physiologic thyroid uptake of free radioiodine.
