**124I-F16SIP** is an iodine-124-labeled human monoclonal antibody fragment (specifically, a small immunoprotein [SIP] or single-chain Fv antibody) that selectively targets the extra-domain A1 of **tenascin-C**, a protein highly expressed in the extracellular matrix of tumors and neovasculature. The drug is used for **PET imaging** (immunoPET) of cancers, including head and neck cancer and various solid tumors, by localizing specifically to tumors expressing tenascin-C. Its mechanism of action as an imaging agent stems from the emission of positrons by iodine-124, enabling visualization through PET scanning. It has shown good bioavailability, selective tumor targeting, and was well tolerated in clinical microdosing studies.
