124I-labeled anti-amyloid mAb 11-1F4 is a radioiodinated monoclonal antibody used for imaging and detecting amyloid deposits in patients with systemic light chain (AL) amyloidosis. The antibody binds specifically to a conformational neoepitope contained within the first 18 amino acids of misfolded human immunoglobulin light chains. When labeled with the positron-emitting isotope 124I, it can be used for positron emission tomography-computed tomography (PET/CT) imaging to visualize amyloid deposits in patients. The antibody retains its immunoreactivity after radioiodination and has shown specific localization to amyloid deposits in experimental models. The unlabeled version of this antibody (11-1F4, later designated as CAEL-101 or anselamimab) has demonstrated the ability to promote phagocytic destruction and clearance of amyloid deposits.
