125I-HE3-G3 is a radiolabeled designed ankyrin repeat protein (DARPin) developed for the in vivo molecular imaging of human epidermal growth factor receptor 2 (HER2) expression. It consists of the G3 DARPin, which binds with picomolar affinity to domain IV of the HER2 ectodomain, modified with an N-terminal histidine-glutamate (HE)3 tag to reduce non-specific background liver uptake, and radiolabeled with Iodine-125 (125I). Developed by researchers at University College London (UCL) Cancer Institute, Barts Cancer Institute, and the University of Zurich, 125I-HE3-G3 is designed to assess HER2 status in breast cancer and other HER2-positive malignancies, enabling non-invasive patient stratification and monitoring of treatment response.
