Overview

This is a combination therapy consisting of 13-cis-retinoic acid (also known as isotretinoin), a vitamin A derivative (retinoid), and interferon alpha (IFN-α), a cytokine with immunomodulatory properties. Both agents have demonstrated antitumor activity through different but potentially synergistic mechanisms. - **Mechanism of Action:** - 13-cis-retinoic acid modulates gene expression by binding to retinoic acid receptors, leading to inhibition of cell proliferation, induction of differentiation, and antiangiogenic effects in malignant cells. - Interferon α binds to the type I interferon receptor on target cells, activating the JAK-STAT pathway and resulting in antiproliferative effects, enhanced immune response against tumor cells, and antiangiogenic activity. The combination has shown efficacy particularly in advanced squamous cell carcinoma of the skin—especially when standard local therapies have failed or are not feasible—and has also been studied in other cancers such as pancreatic adenocarcinoma. Clinical trials report high response rates for advanced local cutaneous squamous cell carcinoma; however, efficacy is lower for metastatic disease or non-squamous histologies[1][2][4][5][6]. Toxicities are generally manageable but may include fatigue (from both drugs) and hematologic side effects (mainly from IFN-α)[1][2].
