131I-cG250 is a radiolabeled chimeric monoclonal antibody targeting the carbonic anhydrase IX (CAIX) antigen, which is highly expressed on clear cell renal cell carcinoma (RCC) cells. The drug consists of the cG250 antibody conjugated to radioactive iodine-131, enabling both targeted delivery of radiation to tumor cells and imaging. Its mechanism involves specific binding to CAIX on tumor cells, followed by internalization and localized emission of beta radiation from iodine-131, resulting in cytotoxicity primarily within the tumor tissue while sparing most normal tissues. Clinical studies have demonstrated high uptake in RCC tumors with relatively low immunogenicity compared to murine G250 antibodies. The primary indication has been for unresectable or metastatic clear cell RCC[2][3][4][5][6][7].
