131I-J591 is a radiopharmaceutical therapy composed of the monoclonal antibody J591, which specifically targets the extracellular domain of **Prostate-Specific Membrane Antigen (PSMA)**, labeled with the beta-emitting radioisotope **iodine-131 (131I)**. PSMA is a type II integral membrane protein that is highly overexpressed on the surface of prostate cancer cells and the neovasculature of many nonprostate solid tumors, but has limited expression in normal tissues. Upon binding to the extracellular domain of PSMA, the 131I-J591 complex is internalized by the cancer cells, where the iodine-131 isotope emits beta particles that cause lethal DNA damage. Developed primarily for metastatic castration-resistant prostate cancer (mCRPC), clinical trials demonstrated its ability to accurately target metastatic sites in bone and soft tissue. However, further clinical development of the 131I-labeled version was largely superseded by versions using radiometals (such as 177Lu-J591 or 90Y-J591) because iodine labels undergo rapid intracellular dehalogenation and washout, whereas radiometals are more effectively retained within the tumor cells.
