131I-NM-02 is a targeted radionuclide therapy (TRNT) agent consisting of the anti-HER2 nanobody NM-02 labeled with the radioactive isotope iodine-131 (131I). Developed by NanoMab, this radiopharmaceutical is designed to treat HER2-positive malignancies, particularly breast cancer. The NM-02 nanobody is a single-domain antibody fragment that provides high affinity and specificity for the human epidermal growth factor receptor 2 (HER2) while maintaining a small molecular size (~15 kDa). This small size allows for rapid tissue penetration and fast systemic clearance, which helps to maximize the radiation dose to the tumor while minimizing exposure to healthy organs. The iodine-131 isotope serves as the therapeutic payload, emitting beta radiation that causes cytotoxic DNA damage in HER2-expressing cells. Preclinical studies have demonstrated that 131I-NM-02 effectively inhibits tumor growth and improves survival in HER2-positive xenograft models with favorable biodistribution and organ compatibility.
