131I-PAM4 is a radioimmunoconjugate consisting of the monoclonal antibody PAM4 (also known as clivatuzumab), which specifically targets a unique epitope on human MUC1 glycoprotein, conjugated with the radioactive isotope iodine-131 (^131I)[1][2]. PAM4 exhibits high specificity for pancreatic cancer tissues and precursor lesions, allowing for selective tumor targeting. Upon intravenous administration, 131I-PAM4 delivers beta-emitting radiation directly to tumor cells, facilitating radioimmunotherapy by inducing localized cytotoxicity, while sparing most normal tissues. Preclinical and early phase clinical studies have demonstrated its ability to localize to primary and metastatic pancreatic tumors, showing antitumor efficacy and tumor regression. It has also been explored in combination with chemotherapeutic agents such as gemcitabine to enhance therapeutic outcomes in pancreatic cancer models[1][2].
