1396-11 is a small-molecule antagonist of the X-linked inhibitor of apoptosis protein (XIAP), belonging to a class of compounds known as "XAntags." It functions by binding to the baculoviral IAP repeat-2 (BIR-2) domain of XIAP, thereby disrupting XIAP's ability to inhibit downstream effector caspases (caspase-3 and caspase-7). This mechanism leads to the induction of apoptosis in cancer cells. Developed in academic settings, 1396-11 has shown therapeutic efficacy in preclinical models of pancreatic cancer, both as a monotherapy and in combination with other treatments like TRAIL, radiation, and gemcitabine, where it acts as an apoptosis sensitizer.
