14-hydroxyclarithromycin is the primary active metabolite of clarithromycin, formed in the liver via CYP3A4-mediated metabolism. It is a semi-synthetic macrolide antibiotic with antibacterial activity against a range of gram-positive, gram-negative, and anaerobic bacteria. Alongside clarithromycin, it exhibits antibacterial effects by reversibly binding to domain V of the 23S rRNA in the bacterial 50S ribosomal subunit, thereby inhibiting protein synthesis and exerting bacteriostatic (and, at higher doses, bactericidal) effects. The metabolite acts synergistically with clarithromycin, especially against certain anaerobic species such as Bacteroides fragilis and Peptostreptococcus anaerobius, and is chiefly detected in plasma as a marker for clarithromycin metabolism and pharmacokinetics[1][2][5][3].
