Ciraparantag (formerly PER977) is a small, synthetic, water-soluble molecule developed by Perosphere Pharmaceuticals (a subsidiary of AMAG Pharmaceuticals) as a broad-spectrum anticoagulant reversal agent. It is designed to rapidly reverse the effects of various anticoagulants, including unfractionated heparin, low-molecular-weight heparin (LMWH), fondaparinux, and direct oral anticoagulants (DOACs) such as factor Xa inhibitors (apixaban, rivaroxaban, edoxaban) and the factor IIa inhibitor dabigatran. The molecule functions through non-covalent charge-charge interactions, binding to the anticoagulant molecules and preventing them from interacting with their biological targets, thereby restoring normal coagulation. The 14C-labeled version, 14C-ciraparantag, is specifically utilized in clinical pharmacology studies, such as mass balance and absorption, metabolism, and excretion (AME) trials, to track the drug's metabolic pathways and disposition in human subjects.
