15-keto latanoprost is a chemically modified derivative of latanoprost, specifically the 15-keto metabolite. It is an isopropyl ester analog that functions as a prostaglandin F2-alpha analog, similar to latanoprost, and is investigated primarily for its ocular hypotensive effects. In preclinical studies, topical 15-keto latanoprost reduced intraocular pressure in primate models of glaucoma, with efficacy comparable to or exceeding standard latanoprost in certain dosing regimens. Its primary mechanism appears to be enhancement of uveoscleral outflow, though in normal monkeys, aqueous humor production and conventional outflow facility were not altered after administration, suggesting a selective effect on the uveoscleral pathway[3][5]. It is used experimentally and as a pharmaceutical standard, not as an approved commercial drug for human therapy.
