161Tb-trastuzumab is a preclinical targeted radiopharmaceutical consisting of the anti-HER2 monoclonal antibody trastuzumab labeled with the beta- and Auger-emitting radioisotope terbium-161 (161Tb). It is designed for targeted radionuclide therapy (TRT) of HER2-expressing solid tumors, such as breast cancer. Terbium-161 emits low-energy beta-minus particles as well as a high abundance of short-range Auger and conversion electrons, which can deliver highly localized, lethal radiation to cancer cells while sparing surrounding healthy tissue. Preclinical studies, including those conducted by Minerva Imaging and the University of Copenhagen, have evaluated 161Tb-trastuzumab both as a monotherapy and in combination with immune checkpoint inhibitors like anti-PD-1 therapy.
