166Ho-boro-macroaggregates (166Ho-HBMA) is a radiopharmaceutical designed for radiosynoviorthesis (RSO), also known as radiation synovectomy. It is used to treat chronic synovitis associated with conditions such as rheumatoid arthritis, gonarthrosis, and psoriatic arthritis. The drug consists of macroaggregates containing the radionuclide holmium-166, which emits high-energy beta radiation (maximum 1.8 MeV) to induce radiation necrosis of pathologically inflamed synovial tissue, and gamma radiation, which facilitates post-therapeutic scintigraphic imaging. The particles, typically around 2.6 μm in size, are designed to be trapped by macrophages and retained within the joint cavity to minimize systemic leakage.
