177-Lu-DOTAGA-IAB57 is an investigational radiopharmaceutical therapy (RPT) developed by ImaginAb for the treatment of small cell lung cancer (SCLC). It consists of the IAB57 minibody—an 80 kDa antibody fragment—conjugated to the chelator DOTAGA and radiolabeled with the beta-emitting radioisotope Lutetium-177 (177-Lu). The minibody is engineered to specifically target Delta-like ligand 3 (DLL3), a Notch ligand that is highly overexpressed on the surface of SCLC cells but has minimal expression in normal tissues. The minibody scaffold is designed to provide optimal pharmacokinetics, offering deeper tumor penetration and faster systemic clearance than full-length antibodies while maintaining high target specificity. Preclinical studies presented at AACR 2024 demonstrated that 177-Lu-DOTAGA-IAB57 achieves significant tumor uptake and effectively reduces tumor size, leading to improved survival in SCLC models with a favorable dosimetry profile well below toxicity thresholds for critical organs.
