177Lu-anti-idiotype sdAb is a preclinical radiopharmaceutical being investigated for the treatment of multiple myeloma, particularly for targeting minimal residual disease (MRD). It consists of a camelid-derived single-domain antibody (sdAb, or nanobody) that is radiolabeled with the beta-emitting isotope Lutetium-177 (177Lu). The sdAb is engineered to recognize the patient-specific idiotype of the membrane-bound paraprotein (M-protein) expressed on the surface of malignant plasma cells. This personalized approach allows for highly selective delivery of ionizing radiation to tumor cells while minimizing off-target effects. The research is primarily led by academic institutions in Belgium, including Vrije Universiteit Brussel (VUB) and UZ Brussel.
