177Lu-atezolizumab is a PD-L1-targeted radioimmunotherapy (RIT) consisting of the humanized monoclonal antibody atezolizumab labeled with the beta-emitting radioisotope lutetium-177 (177Lu). Atezolizumab specifically binds to programmed death-ligand 1 (PD-L1), an immune checkpoint protein often overexpressed on tumor cells and tumor-infiltrating immune cells. By blocking the interaction between PD-L1 and its receptors (PD-1 and B7.1), the antibody component helps restore the anti-tumor activity of T cells. The addition of 177Lu allows for the delivery of targeted ionizing radiation to the tumor site, causing DNA damage and cell death in both the targeted cells and nearby cells via the crossfire effect. This agent is being investigated for its potential to treat immune-desert or desmoplastic tumors, such as pancreatic cancer, particularly when combined with agents like oncolytic viruses that can upregulate PD-L1 expression and improve tumor penetration.
