177Lu-DOTA-daratumumab is an experimental radiopharmaceutical composed of the humanized anti-CD38 monoclonal antibody daratumumab conjugated to the beta-emitting radionuclide lutetium-177 (177Lu) via the DOTA chelator. It is designed to treat multiple myeloma by targeting CD38, a surface protein highly expressed on malignant plasma cells. Once bound, the 177Lu isotope emits beta particles that cause lethal DNA damage to the targeted cells. Preclinical research conducted at the City of Hope has compared this beta-emitter conjugate to alpha-emitting alternatives, noting that while 177Lu-DOTA-daratumumab demonstrates antitumor efficacy, its therapeutic potential is constrained by off-target toxicities, particularly bone marrow suppression, due to the relatively long tissue path length of beta radiation.
