177Lu-EDTMP is a bone-seeking radiopharmaceutical developed for the palliation of pain associated with skeletal metastases, particularly in patients with castration-resistant prostate cancer and breast cancer. It consists of the beta-emitting radioisotope Lutetium-177 complexed with the ligand ethylenediaminetetramethylene phosphonate (EDTMP). The EDTMP ligand has a high affinity for hydroxyapatite, the primary mineral component of bone, which allows the complex to selectively localize at sites of increased bone turnover typical of metastatic lesions. Once localized, Lutetium-177 delivers therapeutic beta radiation to the tumor-bone interface, alleviating pain. Additionally, its low-energy gamma emission (208 keV) allows for scintigraphic imaging and dosimetry, making it a versatile agent for both therapy and monitoring.
