177Lu-FAP-2286 + chemotherapy is a combination regimen under clinical investigation that consists of 177Lu-FAP-2286, a peptide-targeted radiopharmaceutical, combined with chemotherapy agents. 177Lu-FAP-2286 is a conjugate of a fibroblast activation protein (FAP)-binding peptide labeled with the radionuclide lutetium-177, targeting FAP expressed on cancer-associated fibroblasts commonly found in the tumor stroma. The radiopeptide delivers site-specific beta-emitting radiation to FAP-expressing tumors, resulting in tumor cell killing and stromal disruption[1][3][4][10]. Chemotherapy, concomitantly administered, provides additive or synergistic cytotoxic effects by targeting proliferating cancer cells via various mechanisms depending on the specific cytotoxic agents used. The combination aims to improve efficacy through dual targeting of tumor cells (by chemotherapy) and the tumor microenvironment (by 177Lu-FAP-2286). It is under evaluation for the treatment of advanced or metastatic solid tumors, including sarcoma, lung cancer, and other difficult-to-treat cancers[2][7][8].
