Overview

177Lu-J591 is a radioimmunotherapeutic agent consisting of the monoclonal antibody J591 conjugated to the beta-emitting radioisotope lutetium-177 (177Lu). Developed at Weill Cornell Medicine (formerly Weill Medical College of Cornell University), it specifically targets the extracellular domain of prostate-specific membrane antigen (PSMA), a protein highly overexpressed on the surface of virtually all prostate cancer cells, particularly in metastatic and treatment-resistant disease. The J591 antibody serves as a delivery vehicle to bring localized radiation directly to cancer cells throughout the body, inducing DNA damage and cell death while minimizing exposure to healthy tissues. This biologic therapy has been evaluated in Phase I and Phase II clinical trials for metastatic castration-resistant prostate cancer (mCRPC), both as a monotherapy and in combination with other PSMA-targeting agents such as 177Lu-PSMA-617.
