177Lu-octreotate is a radiopharmaceutical consisting of the beta-emitting radionuclide lutetium-177 complexed with the somatostatin analogue octreotate via a DOTA chelator. Developed as a personalized peptide receptor radionuclide therapy (P-PRRT) at CHU de Québec-Université Laval, this specific protocol (NCT02754297) utilizes quantitative SPECT/CT-based dosimetry to tailor the administered radioactivity for each patient. The therapy targets somatostatin receptor type 2 (SSTR2), which is highly expressed in neuroendocrine tumors (NETs). Upon binding and internalization, the lutetium-177 delivers localized ionizing radiation, causing DNA damage and apoptosis in tumor cells. The personalized approach aims to maximize the radiation dose delivered to the tumor while maintaining safe radiation limits for critical organs, particularly the kidneys.
