177Lu-P1B7 is an experimental radiopharmaceutical consisting of the monoclonal antibody P1B7 conjugated to the beta-emitting radioisotope lutetium-177. P1B7 is a T-cell receptor-like (TCR-like) antibody specifically engineered to recognize a unique neoantigen: the KRAS G12C peptide covalently modified by the inhibitor sotorasib and presented on the cell surface by Major Histocompatibility Complex class I (MHC I) molecules. This approach repurposes sotorasib as a "hapten," allowing the antibody to target intracellular KRAS G12C oncoproteins that have been processed and displayed as drug-adducts. Developed by researchers at the University of California, San Francisco (UCSF), 177Lu-P1B7 is intended for use in combination with sotorasib to treat KRAS G12C-mutant cancers, potentially overcoming resistance to inhibitor monotherapy by delivering localized cytotoxic radiation directly to tumor cells.
