177Lu-R3B23 is a targeted radiopharmaceutical consisting of a camelid single-domain antibody (sdAb or Nanobody), R3B23, labeled with the beta-emitting radionuclide Lutetium-177 (177Lu) via a DTPA chelator. The R3B23 nanobody specifically targets the M-protein (paraprotein) expressed by 5T2MM murine multiple myeloma cells. In preclinical research, 177Lu-R3B23 is frequently utilized as a non-targeting control agent to evaluate the specificity and immune-modifying effects of other targeted radionuclide therapies (TRT) in models that do not express the 5T2MM M-protein, such as B16-melanoma. The therapeutic mechanism involves the localized delivery of cytotoxic beta-radiation, which induces DNA damage and subsequent apoptosis in cells expressing the target antigen.
