177lu-rad204 is a radiopharmaceutical composed of a single-domain monoclonal antibody (nanobody) targeting programmed cell death ligand 1 (PD-L1; CD274), conjugated to the beta-emitting radioisotope lutetium-177. The drug is designed for targeted radioimmunotherapy of solid tumors that overexpress PD-L1. By binding to PD-L1 on tumor cells, the nanobody delivers localized radiation from lutetium-177 directly to cancer cells while sparing surrounding healthy tissue. This approach aims to overcome resistance mechanisms associated with current standard-of-care immunotherapies and may be effective in multiple tumor types including non-small cell lung cancer (NSCLC), small cell lung cancer (SCLC), triple-negative breast cancer (TNBC), cutaneous melanoma, head and neck squamous cell carcinoma (HNSCC), and endometrial carcinoma[1][3][5].
