The 18-month MDR-TB regimen (Beijing Chest Hospital) is an investigational combination therapy designed for the treatment of multidrug-resistant tuberculosis (MDR-TB). Developed and evaluated by the Beijing Chest Hospital, this regimen optimizes the treatment duration to 18 months, compared to the traditional 24-month standard. The regimen typically consists of six anti-tuberculosis drugs, including a core combination of amikacin, moxifloxacin, and pyrazinamide, supplemented by three additional agents selected based on the patient's drug susceptibility profile. Potential supplemental drugs include bedaquiline, linezolid, clofazimine, cycloserine, protionamide, clarithromycin, sodium para-aminosalicylate, or ethambutol. The mechanism of action is multi-targeted, involving the inhibition of bacterial protein synthesis, DNA replication, cell wall synthesis, and energy metabolism, aimed at overcoming resistance mechanisms in Mycobacterium tuberculosis.
