**186Re-perrhenate** is a radiopharmaceutical compound consisting of the perrhenate ion labeled with the beta-gamma emitter rhenium-186. It is used primarily as an intermediate for the synthesis of complex radiopharmaceuticals utilized in nuclear medicine, such as for bone pain palliation and cancer brachytherapy. 186Re is produced by neutron activation in nuclear reactors and exists in the form of perrhenic acid or ammonium perrhenate, which can be used to label molecules for targeted delivery. Its mechanism of action is based on the emission of beta particles (maximum tissue penetration ≈ 4.5 mm), which deliver cytotoxic radiation to target tissues, sparing adjacent healthy areas. 186Re-perrhenate can be injected directly or used as a precursor for the preparation of complexes such as 186Re-HEDP (for bone metastases) or 186Re-liposomes (for brachytherapy, notably in glioma). The compound is also suitable for imaging via its gamma emissions[2][3][4].
