188Re-EL is a radiopharmaceutical consisting of the high-energy beta-emitting radioisotope rhenium-188 (188Re) labeled to ethiodized oil (Lipiodol), used primarily for intra-arterial radiotherapy, especially in hepatocellular carcinoma (HCC) and other liver tumors. 188Re emits beta particles (max energy 2.12 MeV) which induce local cytotoxic effects in tumor tissue, with a gamma emission (155 keV) suitable for imaging and dosimetry. Its short physical half-life (16.9 hours) enables therapeutic applications with manageable toxicity and logistical convenience due to on-demand generation from a 188W/188Re generator. The agent is delivered directly to tumor vasculature (typically by hepatic arterial route), exploiting its preferential accumulation in tumor tissue. The primary mechanism of action is targeted beta radiation-induced cell death of malignant cells at the administration site[1][3][5].
