**188Re-ELH** is an experimental locally administered therapeutic radiopharmaceutical formulation for **hepatocellular carcinoma**. It combines beta-emitting rhenium-188 chelated with ECD and dissolved in Lipiodol to form 188Re-EL, with a thermogelling PEG-PLGA-PEG hydrogel that improves local retention and provides sustained radionuclide delivery after direct intratumoral injection. In an N1-S1 rat hepatoma model, the formulation localized predominantly in tumor tissue and produced durable tumor-volume reduction and a 75% long-term survival rate; it remains a preclinical investigational approach rather than an approved therapy.
