Drug intelligence / Profile preview
**188Re-HDD + lipiodol** is a combination radiopharmaceutical designed for selective internal radiation therapy of unresectable or untransplantable hepatocellular carcinoma (HCC)[1]. It consists of a complex of radioactive **rhenium-188 (188Re)** bound to **4-hexadecyl-1,2,9,9-tetramethyl-4,7-diaza-1,10-decanethiol (HDD)**, which provides sulfhydryl groups for binding 188Re, and is solubilized in **Lipiodol**, an iodinated poppy seed oil derivative. Lipiodol accumulates preferentially in HCC tumor tissue due to its unique vasculature and hypervascularity, delivering the beta-emitting rhenium complex directly to malignant cells following intra-arterial (usually hepatic artery) injection. The emitted beta radiation causes localized cytotoxic damage to tumor cells while sparing most surrounding healthy liver tissue[1][3]. 188Re-HDD + lipiodol is considered a generic, cost-effective local radiotherapeutic, with a short half-life (17 hours) allowing for hospital convenience and repeated dosing[1]. It is particularly intended for advanced HCC patients not eligible for surgery or transplantation, including those with portal vein thrombosis. The agent is available under the brand name **REK-1** in India and is under continued development, especially in Asia[1].
